Black and <br /> Hispanic or Latino residents of Indiana are more likely to be living with HIV/AIDS, and the prevalence of <br /> HIV/AIDS is much more common among males than females. There were 12 individuals with HIV/AIDS <br /> that were homeless according to the 2024 Point In Time Count. <br /> Health Plus Indiana works with local individuals with HIV/AIDS to provide care coordination and housing. <br /> AHealth Plus Indiana receives ESG funding for Rapid Rehousing and emergency support services,and also <br /> provides Transitional Supportive Housing, state-based HOPWA housing, and permanent supportive <br /> housing. Health Plus also conducts outreach. <br /> The Center for the Homeless coordinates activities to assist homeless individuals and families in St.Joseph <br /> County and the City of South Bend. In addition to emergency and transitional shelters and permanent <br /> supportive housing,the Center for the Homeless provides adult self-sufficiency training,children's support <br /> and developmental services,and mental health counseling to homeless persons in the area.They also partner <br /> with the Notre Dame Legal Clinic for legal representation for the homeless, though the Legal Clinic's <br /> resources are limited. <br /> Availability of services targeted to homeless persons and persons with HIV and <br /> mainstream services <br /> 0MB Control No:2506-0117(exp.07/31/2025) 141 <br />
