Version 6-8-2017 <br /> 18 <br /> <br /> <br />APPENDIX "A" <br /> <br /> <br />SERVICES TO BE FURNISHED BY CONSULTANT: <br /> <br />In fulfillment of this Contract, the CONSULTANT shall comply with the requirements of the appropriate <br />regulations and requirements of the Indiana Department of Transportation and Federal Highway Administration. <br /> <br />The CONSULTANT shall be responsible for performing the following activities: <br /> <br />A. Personnel <br />For the fulfillment of all services outlined in Section B below, the CONSULTANT will provide a full-time <br />Resident Project Representative, Inspectors, Engineering Support, Clerical, and Accounting personnel as <br />required. <br />The qualifications of personnel provided by the CONSULTANT are subject to approval by the LPA <br />and INDOT. <br /> <br />The Resident Project Representative will take direction from and report to the INDOT Area Engineer <br />and the LPA’s Project Coordinator on all matters concerning contract compliance and administration. <br /> <br />B. Description of Services <br />1. Review the Contractor’s construction schedule and provide the LPA with documentation <br />concerning acceptability. <br />2. Review Contract documents, verify existing site conditions, attend pre-construction <br />conferences, organize progress meetings, and any other job conferences as required and provide <br />a written record of the meeting minutes. <br />3. Serve as the project liaison by working principally through the Contractor’s field <br />Superintendent or other designated authority. Deviations from the plans or specifications will be <br />reported to the appropriate LPA or INDOT representative. <br />4. Obtain from the Contactor a list of proposed suppliers and sub-contractors and obtain sufficient <br />information for the proper execution of work. <br />5. Furnish equipment necessary to sample and test materials in accordance with the frequency of <br />sampling requirements. <br />6. Obtain field samples of materials as required and deliver them to the appropriate laboratory or <br />testing facility. <br />7. Shop and Falsework Drawings: <br />a. Document submittals and check for completeness, then forward to the proper personnel for <br />review and approval. Forward approved documents and maintain copies within the contract <br />file. <br />b. Check construction for compliance with the approved drawings and specifications and alert <br />the Contractor of materials or equipment which are not approved for their intended use. <br />Advise the LPA or INDOT when submissions will not be approved or when it is necessary to <br />disapprove work. <br />8. Review of Work, Inspection and Tests: <br />a. Conduct on-site inspections of the work for conformance with plans and specifications <br />b. Provide the necessary testing of materials as prescribed by the frequency of testing manual. <br />c. Accompany authorized visitors to the project and provide a detailed report of the site visits. <br />9. Collaborate with the Engineer of Record with suggestions for modifications to the project <br />drawings and/or specifications. <br /> <br /> <br /> <br />
