1.Pursuant to Municipal Code Article 9, Section 20-113.1, any person eligible by virtue <br />of a physical or mental disability that substantially limits one or more of the major life <br />activities of such individual as defined by the Americans with Disabilities Act, shall <br />present to the Board of Public Works a written application for a permit entitling <br />that person to the exclusive use for a period of one calendar year, or less, from the <br />date of issue of one parking space adjacent to or directly across the street from the <br />applicant's place of employment. The application must be filled out completely. The <br />cost per month is $45.00. <br />2.The application shall include a letter from a qualified physician on their letterhead <br />verifying that the applicant is a physically disabled person. <br />3.The application for parking at a place of employment shall include evidence of <br />approval by the employer as well as the information regarding the applicant's daily <br />work schedule. No applicant shall be granted a permit without first demonstrating <br />to the Board that the individual is a full-time employee. <br />4.The application must also provide verification from the individual's supervisor at <br />their place of employment attesting to the fact that the employer has exhausted <br />all efforts at providing parking for the individual. No applicant shall be granted a <br />permit unless the applicant's employer provides the board with information that the <br />applicant's employer is unable to provide the applicant with any other parking <br />opportunities comparable to that which the City is able to provide in terms of <br />distance from the applicant's place of employment. The burden is on the applicant <br />and the applicant's employer to demonstrate to the Board that the applicant's <br />employer is unable to provide the applicant with a parking area that adequately <br />meets the needs of the applicant. <br />5.The application must be renewed on a yearly basis, with applications due to the <br />Board of Works by December 1 of the previous year and all required documents <br />attached. <br />6.Approval is subject to availability. <br />Return Application and doctor’s letter to: <br />City of South Bend Engineering Dept. <br />227 W.
