Page 1 of 1 <br />INTER-OFFICE MEMORANDUM <br /> DEPARTMENT OF PUBLIC WORKS <br /> DIVISION OF ENGINEERING <br />TO: Theresa Heffner, Clerk <br />Board of Public Works <br />FROM: Zach Hurst, PE, Sr. Project Engineer <br />SUBJECT: Change Order No. 6 - Project No. 122-034 <br />Fire Station #8 Replacement <br />DATE: 11/05/2024 <br />Change Order No. 6 for the subject project addresses the following items: <br />• Occupancy Items: $3,990.00 <br />o These are plan omissions that were discovered during the final Building <br />Department inspection to obtain occupancy. Items such as new bollards, <br />painting, and locksets. <br />•Dedication Plaque:$5,335.00 <br />o This is an owner-initiated change to install a bronze dedication plaque. <br />•Additional Arborvitae:$2,300.00 <br />o This is an owner-initiated change to install additional arborvitae plantings. <br />•Change to no-mow grass:$440.00 <br />o This is an owner-initiated change to switch to no-mow grass on the small <br />southwest hill. <br />•Additional Sidewalk Replacement: $4,276.00 <br />o This is an owner-initiated change to replace some of the deteriorated sidewalk <br />along Twyckenham Drive, as well as provide accessible parking. <br />•Additional Interior Signage:$1,222.00 <br />o This is an owner-initiated change to install additional interior signage within the <br />firehouse noting different room names and numbers. <br />•Digital Sign:$14,916.00 <br />o This is an owner-initiated change to add a digital sign to the front yard, similar to <br />Station #9. <br />•Additional Curb Replacement:$4,869.00 <br />o This is an owner-initiated change to replace curb along Twyckenham Drive to <br />facilitate accessible parking. <br />•Railing Deduct:($4,541.00) <br />o This is a deduct to reflect the Fire Department choosing a less expensive railing <br />type inside the station. <br />•TOTAL:$32,807.00 <br />This change order does not affect the existing contract completion date of September 19, 2024. <br />Funding for this change order is available through the Fire Department’s capital fund.
