PETITION FOR REZONING <br />TO: Common Council of the City of South Bend, Indiana. <br />Date Filed Application Number <br />Date Area Plan Commission Received <br />The undersigned hereby makes application and petitions <br />the Common Council of the City of South Bend, Indiana, to amend the <br />Zoning Ordinance as herein requested. <br />(1) The property sought to be rezoned is the Saint Joseph's Medical <br />Center Complex, located in the 700 and 800 Blocks of East <br />Madison and East Cedar Streets, South Bend, Indiana. <br />(2) Name of property owner: Saint Joseph's Medical Center, Inc., <br />811 East Madison Street, South Bend, Indiana 46617; telephone: <br />237 -7222. <br />(3) It is desired and requested that this property be rezoned from <br />B- Residential, "B" Height and Area to B- Residential, "G" Height <br />and Area. <br />(4) This rezoning is requested so as to allow the following uses: <br />Hospital, physicians' offices and related uses. <br />(5) Attached is a copy of (a) legal description of the property; <br />(b) a list of names and addresses of all property owners within <br />300 feet of the petition property; (c) self - addressed, stamped <br />envelopes of all property owners within 300 feet of the petition <br />property.
