Attachment A — General Trades: <br />BCRC LETTER HEAD <br />Letter of Notice for Random Testin <br />Dear <br />You have been selected by BCRC's computer generated random selection to take <br />a random drug test for BCRC. You must be tested no later than midni ht da of <br />Month 20 Our computer program selects the names of BCRC cardholders for <br />a random each month. You can be selected up to twice in a twelve month period. <br />In order to stay in compliance with BCRC and possess a clear (CL) BCRC card <br />status you will be required to report to an approved collection site within seven (7) <br />days from the date on this "Notice of Random Testing" letter the date on this letter <br />is included in the seven days}. Please verify that the BCRC card number in the <br />upper right hand corner of this notice matches your BCRC card number. If it does <br />not match your BCRC card number, please notify our office immediately! <br />this random test is to be taken on your own time at any of the BCRC approved <br />collection sites identified on the enclosed collection site list. You will be mailed a <br />$30.00 stipend for the time necessary to secure random testing only if your test is <br />taken within the allotted seven (7) days of this notice. NO EXCEPTIONS! Failure <br />to take your test by the date outlined above will result in your BCRC card status <br />being deemed "Not Available" (NIA) and will require that you contact the BCRC <br />office for instructions on the required procedure(s) for correcting the "NIA" status. <br />In the event you are taking prescribed medication(s), DO NOT take them to the <br />collection site. You will be contacted by our Medical Review Office (MRO), who <br />will need your prescription information for the verification process. Please provide a <br />legible and accurate telephone number on all forms at the collection site. <br />Remember, it is your responsibility to keep your contact information current with <br />BCRC.
