Alcohol and Drug Employee's Gertified Receipt <br />This is to certify that I have been provided educational materials required by 9382.601 and my employer's <br />policies and procedures with respect to meeting the Part 382 requirements. The materials include detailed <br />discussion of the following items: <br />1. The designated person to answer questions about the materials. <br />Wl Office: Mike Frailing MN Office: Brian Baumann <br />2. The categories of drivers subject to Part 382. <br />ALL drivers are subject to regulations. <br />3. Sufficient information about the safety-sensitive functions and periods of the workday that <br />compliance is required. <br />All loading, unloading operations, driving, and machinery operation is considered Safety Sensitive. All field <br />employees are required to be in compliance with the regulations as outlined in the FMCSR'382 and FMCSR <br />'40. <br />4. Specific information concerning prohibited driver conduct. <br />No drug use or alcohol consumption is permitted while on duty. <br />5. Circumstances under which a driver will be tested. <br />All employees are subject to: Pre-employment, Reasonable Cause, Post Accident (any accident in a company <br />vehicle or on company time), Random, and Follow up. <br />6. Test procedures, driverprotection and integrityof the testingprocesses, and safeguarding the validity <br />of the test. <br />Please see FMCSR'382.303 Subpart D. <br />7. The requirement that tests are administered in accordance with Part382 <br />All employees are subject to testing <br />8. An explanation of what will be considered a refusal to submit to a test and the consequences. <br />Refusal of any test is the same as a positive result. <br />9. The consequences for Part382 Subpart B violations including removal from <br />Safety-sensitive functions and Part 40, Subpart 0 procedures. <br />TERMINATION <br />10. The consequences for drivers found to have an alcohol concentration of 0.02 or greater but less than <br />0.04. <br />Suspension for one day as pursuant to '382.505 <br />1 1. Information on the effectss of alcohol and controlled substances <br />Refer to handout entitled Druq and Alcohol Testinq: Traininq and Awareness. which is issued during <br />orientation. <br />APPENDIX K
