COMMUNICATIONS <br />Applicants will be advised in writing of Visu-Sewer's Controlled Substances/Alcohol Policy <br />for drug testing as a condition of employment at the time the applicant is given an <br />application for employment(APPENDIX A - APPLICANT NOTIFICATION). The Controlled <br />Substances/Alcohol Policy will be provided to the applicant upon request. The applicant <br />will be required to sign an acknowledgment of notification of Visu-Sewer's Controlled <br />SubstancesiAlcohol Policy (APPENDIX B - ACKNOWLEDGMENT OF NOT1F|CATION). <br />Applicants for safety sensitive positions requiring a commercial drivers license will be <br />requested to furnish Visu-Sewer with all information concerning prior participation in a drug <br />and alcohol testing program (APPENDIX C - SAFETY PERF)RMANCE H|STORY <br />REcoRD (Dor REGULATED EMPLjYEES and APPENDIK E - PREVtous PRE- <br />EMPLOYMENT EM?LOYEE ALCOHOL AND DRUG IESI STATEMENI-). <br />Employees newly hired by Visu-Sewer will be given the Controlled Substances/Alcohol <br />Policy on their first day of employment. During the employee's orientation, the Policy will <br />be reviewed with the new employee and any questions the employee may have regarding <br />the Policy will be answered. An acknowledgment of receipt and review of the Policy will be <br />signed jointly by the employee and a representative of Visu-Sewer's management as a <br />commitment to comply and abide by the Policy as established. (APPENDIK D - <br />ACKNOWLEDGMENT OF RECEIPT AND REVIEW <br />Current employees will be issued the Controlled Substances/Alcohol Policy as it is <br />established and periodically updated. Managers and supervisors will review the Policy with <br />the employees and answer any questions the employees may have regarding the Policy. <br />An acknowledgment of receipt and review of the Policy will be signed jointly by the <br />employee and a representative of Visu-Sewer's management as a commitment to comply <br />and abide by the Policy as established. (APPENDIK D - A)KNjWLEDGMENT oF <br />RECEIPT AND REVIEW <br />The Controlled Substances/Alcohol Policy will be reviewed at safety meetings to <br />communicate Visu-Sewer's commitment to the Policy. <br />Amendments and updates to the Policy will be provided and distributed to employees <br />through various methods including but not limited to distribution of documents at safety <br />meetings, postings on bulletin boards and in payroll check distribution. <br />Visu-Sewer will communicate and provide this Policy to the business partners associated <br />with Visu-Sewer including but limited to customers/clients, prime contractors, <br />subcontractors, vendors, employment resources and recruiters and other interested <br />ind ividuals and organizations. <br />2
