Jefferson Blvd. <br />South Bend, IN 46601 <br />4777 W LINCOLN AVE WEST MILWAUKEE WI 53219 <br />414 672 4244 the mrt)group net <br />RE: Response Statement Letter for Items Listed Within Your Pre -Qualification <br />Checklist for Bidders That Are Not Pre -Qualified <br />Dear Mr. Hurst/City of South Bend, <br />The MRD Group provided you with a complete Pre -Qualification Package previously for <br />the submission of our bid for the South Bend Medical Foundation which assume was <br />satisfactory as we were chose as the winning bidder for this project and are current out <br />on site performing this project. <br />We did not, however, receive a letter from the City of South Bend saying that we are on <br />the Pre -Qualified Bidders List. <br />With this in mind, I am submitting all the information that is required for the Pre -Qualified <br />Bidders as is required by the bid specification. <br />Please find attached: <br />1. Required Business Verification documentation from the State of Indiana dated <br />within 60 days of our submission. <br />2. Statement of Proof of Competency & Qualification of Bidder <br />3. List of projects of Similar size and work scope. <br />4. The Demolition of the South Bend Medical Foundation site will be the first job <br />performed in Indiana where the MRD Group will be employing Construction Craft <br />Demolition.
