APPLICANT NOTIFICATION OF VISU.SEWER'S <br />CONTROLLED S CES/ALCOHOL POLICY <br />The information presented below is a summary of Visu-Sewer's Controlled Substances/Alcohol Policy. A complete copy of <br />Visu-Sewer's Controlled Substances/Alcohol Policy will be provided to the applicant upon request. <br />Policv <br />Visu-Sewer prohibits employee use, possession, purchase, manufacture, sale, transfer, distribution, or <br />dispensing of illegal substances at any time; use of alcohol on the job, and use of legal drugs or alcohol that <br />would impair the employee's ability to work. <br />Statement <br />Visu-Sewer has a responsibility to provide a safe and healthy workplace for its employees. The Company <br />recognizes that the use and/or abuse of illegal/legal drugs and/or alcohol can have a significant impact on the <br />workplace in terms of safety. Visu-Sewer has established the Controlled Substances/Alcohol Policy to protect <br />the safety of its employees from those individuals who use or are impaired by drugs and/or alcohol on the job. <br />lmplementation of Proqrams <br />To demonstrate Visu-Sewer's commitment to enforcing the Policy, Visu-Sewer has established and <br />implemented a detection program to ensure compliance with the Controlled Substances/Alcohol Policy. Visu- <br />Sewer expects that all employees will fully and willingly participate in the programs with the knowledge that a <br />safe, healthful and productive work environment is to the benefit of all. <br />Detection Prooram <br />The detection program establishes procedures to provide compliance with the Policy <br />Tests Required <br />Drug and/or alcohol testing is required as a part of pre-placement examination. DOT physicals, required for <br />employment as a driver of company commercial motor vehicles, will include drug and alcohol testing. <br />Consent To Testinq <br />Prospective employees must submit to pre-employment drug and alcohol testing. Applicants will be required <br />to complete and sign a form by which he/she consents to and authorizes testing and disclosure of test results <br />to Visu-Sewer. As established by the U.S. Department of Transportation, Federal Highway Administration, <br />Federal Motor Carrier Safety Regulations, implied consent to alcohol testing '383.72 states that any person <br />who holds a CDL shall be deemed to have consented to such testing as is required of him/her by any State or <br />jurisdiction in the enforcement of '383.51 (b) (2) (i) and '392.5 (a) (2). Consent is implied by driving a <br />commercial motor vehicle. <br />Conditions Of Emplovment <br />Visu-Sewer will enforce this Policy as a condition of employment. Employees will abide by the terms of this <br />Policy as a condition of employment. Employees will be subject to drug and/or alcohol testing as set forth in <br />this Policy. Drug and alcohol testing is mandatory for Visu-Sewer employees as a condition of employment. <br />Any job offer is contingent upon, among other things, a negative test. Prospective employees will be notified <br />in advance of said drug and alcohol test. lf the prospective employee refuses to submit to the drug and <br />alcohol test, consideration for employment with Visu-Sewer will immediately cease. A prospective employee <br />will not be allowed to begin work at Visu-Sewer until the results of the drug and alcohol test are known. lf the <br />results of the test are positive, the prospective employee will be denied employment. <br />APPENDIX A
