Klosinski, P.E., Assistant City Engineer <br />SUBJECT: 117-059B South Well Field Water Treatment Plant and Distribution <br />System Improvements – Contract B – Water Main (PROJ00000082) <br />Agenda Request – Change Order No. 1 <br />DATE: April 1, 2024 <br /> <br />The Division of Engineering and Division of Water are issuing this Agenda Request for the Board of <br />Public Works approval of Change Order No. 1 for Project No. 117-059B South Well Field Water <br />Treatment Plant and Distribution System Improvements – Contract B – Watermain. Described, <br />herein, are the additional scope of work items: <br /> <br />Change Order No. 1 – Description of Additional Scope Items <br /> <br />Item 1: Addition of Water Main Insulation at Chippewa Avenue <br />Reason: Unforeseen condition. Existing utilities were discovered to be closer than anticipated <br />requiring a shallower watermain profile through a portion of Chippewa Avenue. <br /> <br /> Item 1 Cost: $10,251.95 See attachment for detail. <br /> <br />Item 2: 24-inch Water Main Repair <br />Reason: Unforeseen condition. During excavation activities at the South Well Field property, a <br />leaking corporation stop and valve were discovered to be leaking beyond the contractor’s <br />excavation. Exploratory excavation uncovered leaking valve and C&E Excavating assisted with the <br />corporation stop replacement since Water Works crews were unavailable due to City crews <br />responding to other water main breaks. <br /> <br /> Item 2 Cost: $ 7,594.70 See attachment for detail. <br /> <br />Item 3: Addition of two (2) 16-inch diameter ductile iron 22.5-degree fittings <br />Reason: Unforeseen condition. The two additional 16-inch ductile iron water main 22.5-degree <br />fittings were required to adjust the vertical profile of the water main at the South Well Field property. <br />Actual depth of the water main differed from the anticipated water main profile. <br /> <br /> Item 3 Cost: 2 x $2,700.00 each = $5,400.00 See attachment for detail. <br /> <br /> <br /> <br /> <br /> <br />
