�ttachment I <br />Letter of Notice for Random T esting <br />Date: (Example: April 8.2016) <br />Re: Testing Procedure for MCA UIndiana State Pipe Trades (ISPTA) Identification Card <br />I�0 <br />Your name has been randomly picked by MCAUISPTA computer generated selection <br />program to take a drug test as soon as possible in order to update your MCAUISPTA <br />identification card. Our computer program selects the names of one -twelfth (1/12) of all <br />participant cardholders for update each month. We want to re-emphasize that your selection for <br />a test is purely a random one. You will be taking the same test, which you initially took when <br />you received your MCAI/ISPTA card. <br />In order to avoid any potential interruption in the status of your card, you must be tested <br />by (Example date: April 15, a016). Tests are to be taken on your own time at any one of the <br />facilities identified on the attached list. You will be mailed a $35.00 MCAUISPTA expense <br />reimbursement check if your test is negative/valid. Reimbursement checks will be void after 60 <br />days from the date of the check. <br />All MCAI/ISPTA participants who are working may have their MCAI/ISPTA card <br />verified with the database administrator in order to determine if their card is "valid" or "not <br />valid" under MCAI/ISPTA Program.
