We <br />have 3 operators, 5 plumbers, one apprentice, 2 maintenance tradesmen and 4 helpers to <br />ensure that we have sufficient employees on staff to complete the work we are bidding on. <br />(v) Individuals who will perform work on the public work project on my behalf will be properly <br />classified as an employee or as an independent contractor under all applicable state and <br />federal laws and local ordinances. <br />(vi) See attached for evidence of participation in apprenticeship and training programs are <br />approved by and registered with the United States Department of Labor's Office of <br />Apprenticeship, or its successor organization. <br />(vii) Written plan for employee drug testing from Local 172 attached. <br />(viii) Statement attached re: utilizing a surety company which is on the Bureau of Fiscal Service " <br />Dept. of Treasury's Listing of approved Sureties". <br />(ix) We do not have any federal, state or local tax liens or tax delinquencies owed to any federal, <br />state or local taxing body in the preceding three years. <br />(i) City of Mishawaka <br />Thank you fdr your time and consideration. <br />Sincerely, <br />
