<br /> PAGE 1 OF 2 APPENDIX “B” <br />APPENDIX “B” <br /> <br />INFORMATION AND SERVICES TO BE FURNISHED BY THE LPA <br /> <br /> The LPA shall furnish the CONSULTANT with the following: <br /> <br /> <br />1.0 Designate in writing a person to act as LPA’s representative with respect to the services <br />to be rendered under this Agreement. Such person shall have complete authority to <br />transmit instructions, receive information, interpret, and define LPA’s policies and <br />decisions with respect to the CONSULTANT’s services for the Project. <br />2.0 Designate an employee as the Employee in Charge responsible to coordinate activities <br />between the CONSULTANT and the LPA and INDOT. <br />3.0 Provide all criteria and full information as to LPA’s requirements for the Project, <br />including design objectives and constraints, space, capacity and performance <br />requirements, flexibility and expandability, and any budgetary limitations. <br />4.0 Furnish all specifications and standard drawings applicable to the project and all criteria <br />for design and details including, but not limited to, signage, highways, structures, grades, <br />curves, sight distances, clear zones, clearances, and design loadings. <br />5.0 Assist the CONSULTANT by placing at CONSULTANT’s disposal all available <br />information pertinent to the Project including, but not limited to, the following: <br />5.1 Previous reports and any other data relative to design or construction of the <br />project. <br />5.2 Available data from the transportation planning process. <br />5.3 Utility plans available to the LPA <br />5.4 As-built and other record plans for the project area. <br />6.0 Arrange for access to and make all provisions for CONSULTANT and Subcontractors to <br />enter upon public and private property as required to perform services under this <br />Agreement. <br />7.0 Furnish approvals and necessary permits from all governmental authorities having <br />jurisdiction over the Project and such approvals and consents from others as may be <br />necessary for completion of the Project. Permit fees shall be paid by LPA at time of <br />submission of said applications. <br />8.0 Examine all studies, reports, sketches, drawings, specifications, proposals and other <br />documents presented by CONSULTANT, obtain advice of attorney, insurance counselor <br />and other consultants as LPA deems appropriate for such examination and render in <br />writing decisions pertaining thereto within a reasonable time so as not to delay the <br />services of CONSULTANT. <br />9.0 Furnish all legal services as may be required for the development of the project. <br />10.0 Provide written approval of completed work phases as described in Appendix “A” of this <br />Agreement. Accomplish reviews and provide written approvals in a timely manner.
