APPENDIX B <br />Employee/Supervisory Positions Subject to Alcohol Testing <br />Individuals Subject to Alcohol Testing <br />The following classifications of jobs are considered a "Covered Employee" subject to alcohol <br />testing under the Trisco Systems, Inc.'s alcohol testing policy: <br />Affected Employees <br />1. (NOTE: All new applicants for employment are subject to drug testing only). <br />2. Any employee who appears to be impaired or is suspected of substance abuse (see <br />reasonable cause) which may cause work impairment or safety considerations, will be <br />tested by a laboratory designated by the Company. In such cases, the employee will be <br />placed on "involuntary" leave of absence immediately. If test results are positive, all <br />benefits. Health insurance will lapse unless the employee is reinstated or provides <br />otherwise in accordance to law. <br />3. Any employee who voluntarily admits to a substance abuse condition will be tested by a <br />laboratory designated by the Company and will be placed on an immediate leave of <br />absence and return to work only after a negative test result is obtained. <br />4. All management personnel, all supervisory personnel, all truck drivers, all sales staff, <br />all field and production or warehouse personnel, and any other employee of Trisco <br />Systems, Inc. including secretarial, clerical and administrative staff, shall be subjected <br />to periodic substance abuse testing. In addition, any applicant to any of these positions <br />may be tested. <br />S. Anyone involved in an accident on the he job that requires medical attention or causes <br />damage to product or facilities. <br />6. Any violation of Company Policies and/or Standards of Conduct and Discipline. <br />NOTE: a) Refusal to take the substance abuse test shall be subject to immediate <br />termination of employment. <br />b) Any employee attempt to cheat on these tests or falsify test results shall <br />be terminated. <br />Therefore, Safety sensitive employees include; <br />1. All Project Managers. <br />2. All Foremen. <br />3. All Production Employees, including warehouse employees. <br />4. All operators of Trisco Systems, Inc., owned vehicles at any time. <br />5. All operators driving any other vehicle on Company business or during Company <br />time. <br />6. All clerical, sales, secretarial, and administrative employees. <br />W. <br />
