12 <br />The urine specimens collected under DOT Regulations may only be used to test for controlled <br />substances designated or approved for testing by the DOT and shall not be used to conduct any <br />other analysis or test unless otherwise specifically authorized by the DOT. <br /> <br />Drug Testing Laboratory <br /> <br />Trisco Systems, Inc. shall use only drug testing laboratories certified by the Department of <br />Health and Human Services under DOT procedures for drug testing. <br /> <br />Preparation for Testing <br /> <br />Trisco Systems, Inc., the collection site, and the certified laboratory shall develop and maintain <br />clear and well documented procedures for collection, shipment, and accessioning of urine <br />specimens under this policy. Such procedures shall include, at a minimum, the following: <br /> <br />1 Utilization of a standard drug testing custody and control form (Form #DOT <br />3900.9 or equivalent); <br />2 Use of a clean, single use specimen bottle that is securely wrapped until filled <br />with the specimen, and the use of a tamper proof sealing system, designed in a <br />manner such as to ensure against undetected opening; <br />3 Use of a shipping container in which the specimen and associated paperwork may <br />be transferred and which can be sealed and initialed to prevent undetected <br />tampering; <br /> <br />Written procedures, instructions and training shall be provided as follows: <br /> <br />a. Under normal circumstances, the Company will contract for and utilize when <br />possible, an independent collection site. The independent collection site shall <br />abide by all procedures, techniques, and methods outlined in 49 CFR Part 40, Part <br />382, and any applicable DOT agency regulation, as well as those outlined in this <br />document. <br />b. When an independent collection site is not available, the Company’s collection <br />procedures and training shall clearly emphasize that the collection site person is <br />responsible for maintaining the integrity of the specimen collection and transfer <br />process, carefully ensuring the modesty and privacy of the donor, and is to avoid <br />any conduct or remarks that might be construed as accusatorial or otherwise <br />offensive or inappropriate; <br />c. The collection site person shall have successfully completed training to carry out <br />this function, or shall be a licensed medical professional, or a technician who has <br />been provided instructions for collection and certifies completion as required in <br />this policy. <br />d. Unless it is impracticable for any other individual to perform this function, a <br />direct supervisor of an employee shall not serve as the collection site person for a <br />test of the employee; and
