Project No. 122-001 Curb and Sidewalk Round 1 (Divisions 3, 4, and 5) <br />Change Order 1 Supplemental Information <br />Attn Board of Public Works: <br />The change order for a decrease in the contract amount for Curb and Sidewalk Round 1 <br />(Divisions 3, 4, and 5) is due to changes in scope and unforeseen conditions during the project increasing <br />the amount of work completed by Premium Concrete. The bulk of the increase in this change order <br />comes from an addition of concrete curb at a location in Division 4 to assist with drainage at this location. <br />Also, additions to concrete sidewalk were made as failing sidewalk adjacent to the project boundaries at <br />several locations were added to the project during the pre-construction walkthrough. <br />Daniel Sexton <br />Project Engineer
