All positive <br />drug or alcohol screens may result in termination, even on the first offense. <br />Per the following: <br />• ICISAT—this testing program is applicable to those employees working in the following <br />areas: <br />Indianapolis, Lafayette, Richmond, Columbus, Bloomington and Terre Haute areas. Contact <br />Human Resources for further information regarding ICISAT testing. <br />BCRC—this testing program is applicable to those employees working in the Griffith and <br />South Bend Indiana areas. Contact Human Resources for further information regarding BCRC <br />testing. <br />Applicable To Both Union And Non -Union Employees <br />Requirements For Submitting Results To CCS — Coalition For Construction Safety <br />• A copy of the chain of custody initiated at the time of collection must be submitted. <br />• Test results documentation must include panel cutoff levels — both screening and <br />confirmation. (Exception — if documentation clearly indicates that the test was conducted <br />according to DOT guideline). <br />• Date the specimen was collected and reported. Must be within the last 12 months. <br />• The MRO (Medical Review Officer) name and signature. <br />• The employee's first name, middle initial, last name, at least the last 6 digits of the <br />employee's SSN. And their date of birth. <br />• Test result and date the result was verified by the M RO. <br />• The name of the SAMHSA certified lab that conducted the testing. <br />
