Klosinski, P.E., Assistant City Engineer <br />SUBJECT: 120-017 CSO 18 Backwater Gate Replacement <br />Agenda Request – Final Completion Affidavit <br />DATE: December 5, 2022 <br /> <br />The Division of Engineering and Division of Utilities request the Board of Public Works approval of <br />the Project Completion Affidavit for Project No. 120-017 CSO 18 Backwater Gate Replacement. <br /> <br />Included are the following documents for closeout: <br /> <br /> Project Completion Affidavit <br /> Maintenance Bond <br /> Final Waiver of Lien <br /> Final Pay Application No. 3 – Release of Retainage <br /> <br />Summary of Project – Final Costs: <br /> <br /> Original Contract Price $124,980.00 <br /> Change Orders (None) $ 0.00 <br /> Change Orders, Percentage Change 0.00% <br /> Final Contract Price: $124,980.00 On Budget <br /> <br />This project was completed on budget. <br /> <br />There was a change in the MWBE Utilization Plan that occurred during construction. City <br />Engineering Team directed Selge Construction to remove the old concrete lid from CSO 18 for <br />use at the CSO 27/28 collapse and repair project to save on costs for the CSO 27/28 manhole <br />structure repairs. This allowed for re-use of the material for the repair project. <br /> <br />Consequently, V&R Trucking had originally been scheduled to haul this item away from CSO 18 <br />for demolition, but the lid was hauled by Selge Construction to the CSO 27/28 site due to <br />scheduling needs for the CSO 27/28 project. This resulted in a minor change to the MBE <br />Utilization Plan. This change is being recognized with the Final Completion documents. Refer <br />to attached memorandum from Selge Construction Company explaining the adjustment to the <br />MBE Utilization Plan. <br /> <br />Closeout documents are submitted for approval at the 12/05/2022 Board of Public Works <br />Meeting.
