Disciplinary Action for Policy Violation <br />Applicants <br />If the final result of a pre-employment drug screen is positive, the applicant will not be employed. <br />Reemployment of an employee who has previously failed a drug screen is at the discretion of the employer. <br />Contractors, Subcontractors, Vendors, Their Employees' Agents, Servants or Representatives. <br />No search, inspection or drug test will be conducted without written consent. However, anyone who <br />refuses to provide such written consent and does not fully cooperate with this policy will be subject to <br />disciplinary action up to and including removal from the job or job site, as may be appropriate. <br />Preliminary findings of a policy violation may require that the individual involved be suspended from <br />the job pending the results of the company investigation. <br />If the final result of a "random", "for cause" or "post -accident" drug screen is positive, the individual <br />will be permanently barred from job. <br />Administrative Guidelines <br />• All employees will be informed regarding this policy at the time of employment. Additionally it will <br />be discussed periodically at "tail gate" safety meetings. <br />• In the even an employee or terminated employee requests a review be conducted regarding his/her <br />positive test result, the laboratory will conduct a retest of a portion of the same sample previously <br />tested and will <br />make the results of the retest available to the employee or terminated employee provided the request is <br />made within 12 months following the initial test. The employee who requests a retest will be responsible for <br />all costs involved. <br />• A good faith effort will be made to advise any employee who receives a positive test result via a <br />meeting <br />with the employee's Supervisor. <br />Target Substances <br />As a minimum, the following substances and detection levels shall be tested for: <br />Alcohol .04 BAL Amphetamines 1000 NG/ML <br />Opiates 2000 NG/ML Cannabinoids 50 NG/ML <br />Cocaine 300 NG/ML Phencyclidine 25 NG/ML <br />"Concentrations at or in excess of the above levels shall be conclusive proof of unacceptable levels <br />of unauthorized, prohibited, illegal or controlled substances. <br />7 <br />
