Klosinski, P.E., Assistant City Engineer SUBJECT: 120-026 Airport Annexation Area Water Main Extension Agenda Request – Final Completion Affidavit DATE: November 14, 2022 The Division of Engineering and Division of Utilities request the Board of Public Works approval of the Project Completion Affidavit for Project No. 120-026 Airport Annexation Area Water Main Extension. Included are the following documents for closeout:  Project Completion Affidavit  Maintenance Bond  Final Waiver of Lien Summary of Project – Final Costs: Original Contract Price $313,196.40 Change Orders (Net Deduct) ($ 75,740.30) Change Orders, Percentage Change (24.18%) Final Contract Price: $237,456.10 Under Budget This project resulted in completion under budget as a result of a field change that allowed for the water main to be installed south of existing utilities along US 20 and outside of the shoulder area that would have been impacted by the original alignment for the water main. The savings for this project were the result of lower hot mix asphalt, tack coat, and compacted aggregate materials that did not need to be installed. This resulted in both material and labor savings on the project. This water main will serve the new South Bend Chocolate Company and Dinosaur Museum site at the intersection of US 20 and Olive Road. Closeout documents are submitted for approval at the 11/22/2022 Board of Public Works Meeting. For questions regarding the subject agenda request, contact Jacob M.
