Klosinski, P.E., Assistant City Engineer <br />SUBJECT: 120-026 Airport Annexation Area Water Main Extension <br />Agenda Request – Final Completion Affidavit <br />DATE: November 14, 2022 <br /> <br />The Division of Engineering and Division of Utilities request the Board of Public Works approval of <br />the Project Completion Affidavit for Project No. 120-026 Airport Annexation Area Water Main <br />Extension. <br /> <br />Included are the following documents for closeout: <br /> <br /> Project Completion Affidavit <br /> Maintenance Bond <br /> Final Waiver of Lien <br /> <br />Summary of Project – Final Costs: <br /> <br /> Original Contract Price $313,196.40 <br /> Change Orders (Net Deduct) ($ 75,740.30) <br /> Change Orders, Percentage Change (24.18%) <br /> Final Contract Price: $237,456.10 Under Budget <br /> <br />This project resulted in completion under budget as a result of a field change that allowed for <br />the water main to be installed south of existing utilities along US 20 and outside of the shoulder <br />area that would have been impacted by the original alignment for the water main. The savings <br />for this project were the result of lower hot mix asphalt, tack coat, and compacted aggregate <br />materials that did not need to be installed. This resulted in both material and labor savings on <br />the project. <br /> <br />This water main will serve the new South Bend Chocolate Company and Dinosaur Museum site <br />at the intersection of US 20 and Olive Road. <br /> <br />Closeout documents are submitted for approval at the 11/22/2022 Board of Public Works <br />Meeting.
