<br /> <br />INTER-OFFICE MEMORANDUM <br /> <br /> DEPARTMENT OF PUBLIC WORKS <br /> DIVISION OF ENVIRONMENTAL SERVICES <br /> <br /> <br />TO: Board of Public Works <br />Theresa Heffner, Clerk <br />FROM: Leslie Biek, P.E., Assistant City Engineer <br />SUBJECT: Change Order # 1-3 <br />Olive St Rehabilitation Ph 2 (PN 114-064) <br />DATE: 9/13/2022 <br />Attached are change orders 1-3 for the Olive St Rehabilitation Project (PN 114-064). <br /> <br />Original Contract Amount: $655,280.20 <br />Previously Approved change orders: $0 <br />Percent increase including request: 0.789% <br />New Contract Amount: $660,447.85 <br /> <br />Change order #1: is for the removal of the concrete material that was in the bid specifications. <br />Decrease amount: -$146,080.00 <br /> <br />Change order #2 is for the additional of a different concrete material due to the small quantity of <br />pavement needed for this project. <br />Increase amount: $146,080.00 <br /> <br />Change order #3 is to remove and replace 5 castings due to their existing condition. The original <br />plan was to adjust to grade. <br />Increase amount: $5,167.65 Total ($1,033.53 Local) <br />Total percent increase: 0.789% <br />Revised contract amount $660,447.85 <br /> <br />As this is a federal aid project, INDOT is responsible for the construction contract with Milestone. <br /> <br />It is requested the Board of Public Works approve Change Orders 1-3 for the Olive St <br />Rehabilitation Ph 2 project (PN 114-064) for a total increase of $5,167.65 (Local share of <br />$1,033.53) which is a total increase of 0.789% to the contract price bringing the new <br />contract price to $660,447.85. <br /> <br />Thank you for your consideration of this request. <br /> <br /> <br />
