BOARD OF PUBLIC WORKS <br />AGENDA ITEM REVIEW REQUEST FORM <br />TO: Board of Public Works <br /> <br />FROM: Leslie Biek, P.E., Assistant City Engineer <br />SUBJECT: 2022 CCMG Road Rehab (12-024) <br />Local Roads and Bridges Matching Grant Agreement with INDOT <br />DATE: 7/5/2022 <br />Attached is the INDOT/LPA Agreement for the 2022 Community Crossings Road Rehabilitation <br />project (122-024). INDOT will match up to $1,000,000 for the road reconstruction of City streets. <br />The streets chosen for this grant are in accordance with the City’s “Street Improvement Plan & <br />Strategy”. <br /> <br />INDOT uses DocuSign for their contracts. As a result, INDOT will send the Board President an e- <br />mail when the contract is ready to sign electronically. <br /> <br />Therefore, it is requested the Board of Public Works approve the INDOT/LPA Agreement for <br />the 2022 CCMG Road Reconstruction (122-024) project and authorize the Board President <br />authority to sign the contract on DocuSign when it is available. <br /> <br />Do not sign the attached document. <br /> <br />Thank you for your consideration of this request. <br />
