SCOPE OF WORK For work at Jefferson Middle School, provide all labor and material needed to remove and modify curbs and walks, as described herein, and in the plans and specifications. STATEMENT OF BIDDERS QUALIFICATIONS/AFFIRMATIVE ACTION It is an absolute requirement that each proposal quotation be accompanied by a completed and signed “Statement of Bidders Qualifications/Affirmative Action” form. Statement of Bidders Qualifications/Affirmative Action, Schedule of Liability Requirements, Building Plan and other related documents are attached as referenced. GENERAL CONDITIONS Each contractor shall make a careful examination of this document and attend the pre- quote meeting Monday, May 2, 2022 at 10:00 a.m. at Jefferson Middle School bus loop (South Street) and acquaint themselves with all the conditions before making his proposal. All work to be scheduled through the office of Facilities Management. Work cannot interrupt school operations and must be completed as soon as possible. QUOTATIONS Quotations shall be in a sealed enveloped marked in the upper left hand corner “Quotation for Curb & Walk Replacement – Jefferson Middle School” delivered to: Rahman Johnson, Director of Purchasing South Bend Community School Corporation 215 South Dr. Martin Luther King Jr. Blvd South Bend, IN 46601 by Friday, May 6, 2022 at 2:00 p.m. local time, in the office of Purchasing. Before contract is let, successful contractor must submit a Certificate of Insurance that meets the requirements outlined in this document.
