Viking Painting, LLC <br />Company Drug and Alcohol Policy <br />As a condition of employment, I hereby agree to following rules and regulations <br />pertaining to illegal drugs, alcohol and legally prescribed medical drugs: <br />1. I agree to notify my employer of the need for me to take any prescription <br />drug. <br />2. I agree not to operate any equipment or motor vehicle while taking a <br />prescribed drug that may impair the safe performance of my duties. <br />3. I agree never to drink alcohol when operating equipment on the job. <br />4. I agree never to use any illegal or controlled substance while employed. <br />5. I agree never to report for work while under the influence of alcohol or <br />illegal drugs. I will advise my supervisor upon reporting to work if I am <br />taking medication prescribed by a doctor. <br />6. I agree to take a pre-employment drug test, a drug test after any accident on <br />the job in which employee drug use is likely to have contributed to the <br />incident, and for which the drug test can accurately identify impairment <br />caused by drug use or a random drug test if there is just cause. <br />7. I agree that if I am asked to take a test for illegal drugs or a controlled <br />substance and refuse, that refusal will constitute my immediate voluntary <br />resignation. <br />8.
