Exhibit H 1632-1636 Lincolnway West <br />NSE Case History <br />Case Opened: 08/14/2018 <br />Case Transferred to Jason Miller: 9/27/19 <br />Hearings Held: <br /> 9/20/18 – Repair with deadline of 9/20/18 <br /> 2/26/19 – Demo modified to repair with deadline of 11/29/19 <br /> 1/14/20 – Repair with deadline of 6/19/20 <br /> 10/13/20 – Demo modified to repair with deadline of 11/13/20 <br />o Reverted to demolition after 11/13/20 deadline not met <br /> <br />Exterior Inspections: 48 inspections with photos since case opened <br /> <br />Demolition Overview <br /> <br />Once a property is found to be a risk to public safety, a property may be processed for demolition. <br /> <br />That process is as follows: <br /> Property owner letter - notify the owner of demolition <br /> Environmental Review Checklist completion (if applicable) <br /> Asbestos testing (if able) <br /> Request utilities be removed/retired for demolition <br /> Send demolition quote requests or go through Board of Public Works process (cost and size of <br />project dependent) <br /> Track quotes from contractors <br />o Lowest, responsible quote/bid will be awarded the project <br /> Notify contractors <br /> Send paperwork to winning contractor <br /> Assigned contractor takes over project and completes project in accordance with all specs, <br />local/state/federal regulations, insurance, licensure, notice requirements <br /> NSE monitor progress <br /> NSE verifies project is complete according to specs <br /> NSE receives and pays contractor’s invoice <br /> NSE invoices owner (all costs plus admin demolition fee) <br /> NSE may place a lien on the property for unpaid monies owed <br /> <br />At no time does the city own the property. <br />
