Version 6-8-2017 <br />25 <br />d.Prepare a brief report of the Waters of the US Determination findings including a location map <br />and study boundary shown on a site drawing or aerial photograph. <br />e.Complete a Preliminary Jurisdictional Determination Form for inclusion in the report. <br />L.Public Information/Design Hearing Meeting <br />The CONSULTANT shall assist the LPA in holding a Public Information or a Design Hearing Meeting, <br />if required. The CONSULTANT shall provide the following services as required for the meeting: <br />1.The CONSULTANT shall attend the meeting. <br />2.Prepare materials for the meeting, including displays and handouts. <br />3.Assist the LPA in obtaining a location to conduct the meeting. <br />4.Prepare a PowerPoint presentation to present at the meeting, if necessary. <br />5.Prepare legal notices for publication concerning the meeting location, date, etc., if required. <br />6.Prepare a summary of the meeting minutes for the items discussed. <br />7.If a Design Hearing is required, the CONSULTANT shall prepare transcripts listing all public <br />comments with responses. <br />M.Airport and Railroad Coordination <br />1.An allowance of $18,400.00 has been included for the CONSULTANT to coordinate with the South <br />Bend International Airport (SBIA) and the Northern Indiana Commuter Transportation District <br />(NICTD), as directed and described below. <br />2.The CONSULTANT shall provide railroad coordination with NICTD. Railroad coordination shall <br />be performed in accordance with Chapter 105 of the IDM and shall include the following, as <br />necessary: <br />a.Schedule an on-site meeting with railroad, if necessary. <br />b.Coordination with NICTD for review and approval of preliminary plans for proposed <br />improvements near the railroad. <br />c.Prepare railroad permitting, as required, and coordinate with NICTD for review and approval. <br />Permit fees will be considered a reimbursable expense. <br />d.Monitor the permit approval process and provide any additional information, as necessary, to <br />secure the permits. <br />e.Prepare and coordinate the railroad agreements between NICTD and the LPA, including but <br />not limited to, force account, flagging during field/design services, and flagging during <br />construction. <br />f.Development of the railroad special provision to be included in the letting documents. <br />g.It is assumed that any railroad track design and construction will be performed by the railroad <br />through the Force Account agreement. <br />h.It is anticipated that the storm sewer will cross under the existing NICTD track and be installed <br />via jack and bore method. <br />3.The CONSULTANT shall provide coordination with the South Bend International Airport. Airport <br />coordination shall include the following: <br />a.Contact SBIA to obtain pertinent information concerning, but not limited to, existing SBIA <br />facilities, glide path restrictions across Bendix Drive within the limits of the east-west runway, <br />and restrictions related to construction activities for this project, <br />b.Provide preliminary plans to SBIA for their review and approval, <br />c.Attend up to two coordination/plan review meetings with SBIA to facilitate review and <br />approval of the proposed improvements, <br />d.Prepare SBIA/FAA permitting, as required – including FAA Form 7460-1, and coordinate with <br />review and approval. Permit fees will be considered a reimbursable expense.
