Klosinski, P.E., Assistant City Engineer <br />SUBJECT: 117-132 Wastewater Treatment Plant Electrical Improvements <br />Agenda Request – Change Order No. 2 & Project Completion Affidavit <br />DATE: February 14, 2022 <br /> <br />The Division of Engineering and Division of Utilities recommend the Board of Public Works approval <br />of Change Order No. 2 (No Cost Change) and the Project Completion Affidavit for Project No. 117- <br />132 Wastewater Treatment Plant Electrical Improvements. <br /> <br />Change Order No. 2results in a no cost change in price. <br /> <br />Original Contract: $6,332,340.00 <br />Previous Changes: $ 84,059.00 <br />Contract Sum Prior to Change Order: $6,416,399.00 <br />Change Order No. 2 Amount: $ 0.00 No Cost Change <br />New Contract Sum – FINAL: $6,416,399.00 <br />Total Percent Change by Change Order No.2: 0.00% <br />Total Percent Change for Entire Project – FINAL: +1.33% <br /> <br />Final date for the completion of construction and punch list items is 12/31/2021. Adjustment in time <br />was due to materials and shipment delays as a resulting from COVID-19 restrictions in neighboring <br />states where factory production was heavily impacted to different mandates and local restrictions. <br />The electrical switchgear, motor control centers, and generator production and delivery were <br />delayed and caused installation delays throughout the project that were of no fault to the General <br />Contractor, Martell Electric or its subcontractors. <br /> <br />Martell Electric, LLC has completed all work for this project. <br /> <br />For questions regarding the subject agenda request, contact Jacob M.
