BOARD OF PUBLIC WORKS <br />AGENDA ITEM REVIEW REQUEST FORM <br />TO: Board of Public Works <br /> <br />FROM: Leslie Biek, P.E., Assistant City Engineer <br />SUBJECT: 2021 CCMG Round 2 (121-052) <br />Local Roads and Bridges Matching Grant Agreement with INDOT <br />DATE: 12/6/2021 <br />Attached is the INDOT/LPA Agreement for the 2021 CCMG Rd 2 project (121-052). INDOT will <br />match up to $350,100 for the milling and paving of City streets. The streets chosen for this grant <br />are in accordance with the City’s “Street Improvement Plan & Strategy” approved last year. <br /> <br /> <br />INDOT has switched their contracts to DocuSign. As a result, INDOT will send the Board <br />President an e-mail when the contract is ready to sign electronically. <br /> <br />Therefore, it is requested the Board of Public Works approve the INDOT/LPA Agreement for <br />the 2021 CCMG Round 2 (121-052) project and authorize the Board President authority to <br />sign the contract on DocuSign when it is available. <br /> <br />Do not sign the attached document. <br /> <br />Thank you for your consideration of this request. <br />
