Jefferson Blvd. <br />South Bend, IN 46601 <br />RE: Pre -Qualification Checklist (For Bidders That Are Not Pre -Qualified) <br />Attachment vi <br />With regards to compliance with IC 5-16-13-12 providing or participating in a job <br />training program, the various Union training programs are in compliance with US <br />Department of Labor as well as the Occupational Safety and Health Administration. <br />All of our employees that work in the field are members of one Union or another and <br />receive training thru their corresponding Unions. As part of each collective <br />bargaining agreement, we pay a certain monetary amount per man hour for this <br />training. <br />In addition to that, we have training beyond what the Unions provide. As an example, <br />we have everyone complete CPR and First Aid Training. Then depending on the job <br />class of employee they also receive additional training. We recently had twelve field <br />personnel take an updated trench safety program offered by The Construction <br />Advancement Foundation (CAF) in Portage. The CAF is actually the Union affiliated <br />extension that is responsible for training programs for multiple Unions. Another <br />example ofthe Union training is whereby three of our laborers are actually in training <br />for a week in April near Indianapolis dictated by their Union requirements. Beyond <br />the required Union training, we actually conduct our own Safety Training Day every <br />year where we hire outside instructors to teach various topics from trench safety, <br />confined space, silica training, equipment safety, fire prevention, utility location, etc. <br />Our Safety Training Day is a full eight hour day that all employees attend and like <br />previously stated is over and above what the Union training requires. <br />Feel free to contact me if you have any specific questions regarding this information. <br />Sinnccejrrelly�,'' <br />4 '� <br />Bruce R.
