Klosinski, P.E., Assistant City Engineer <br />SUBJECT: 120-036 WWTP Masonry Improvements Gallery Buildings 1-4 <br />(PROJ00000217; PO-00005683) <br />Agenda Request – Change Order No. 1 <br />DATE: October 5, 2021 <br /> <br />The Division of Engineering and Division of Utilities recommend the Board of Public Works approve <br />Change Order No. 1 for Project No. 120-036 WWTP Masonry Improvements Gallery Buildings 1-4 <br />in the amount of $20,486.00 and a 150-day time extension. <br /> <br />Slatile Roofing and Sheet Metal has submitted Change Order No. 1 which includes the replacement <br />of damaged and heavily corroded steel lintels (unforeseen condition) at Gallery Buildings No. 1, 2, <br />and 4; removal of embedded electrical conduits and wiring at Gallery Building No. 2; and demolition <br />of abandoned boiler piping at Gallery No. 4. <br /> <br />The 150-day time extension accounts for the adjustment in time for a delayed start for the grouting <br />work since the project was awarded late in 2020 and the work could not begin until late winter / <br />early spring 2021 due to temperature constraints on the grout product. There were also materials <br />procurement delays that impacted schedule due to steel supply fabrication schedules. <br /> <br />The funding source for the change order shall be Account 642-06-605-514-442005. <br /> <br />For questions regarding the subject agenda request, contact Jacob M.
