Should the permittee wish to change the Authorized Representative <br />responsible to receive correspondence from the Control Authority a <br />written designation must be submitted to the Control Authority <br />indicating the name of the new Authorized Representative designated <br />to receive compliance correspondence, their position in the company, <br />a statement that they meet the requirements in Part IV (B) (15) of this <br />permit, complete contact information, and an effective date of this <br />designation. <br />
