■ At the start of the academic year, the training company will submit to ABC a class schedule with <br />a starting date, enrollment forms for each student accompanied by a driver's license and proof of <br />high school graduation, a DOL form, and an NCCER Registration and Release Form <br />■ At the conclusion of the academic year, the Education Contact will submit to ABC, all sign in <br />sheets, work mandated make up forms, performance profiles and written tests. <br />ABC will: <br />■ NCCER certify instructors teaching classes for the Michiana Contracting in-house apprenticeship <br />program; <br />• order all craft textbooks; <br />■ provide orientation materials, verify attendance in Flashpoint, grade reports, wage letters, DOL <br />information, NCCER credentials, and invoices; <br />■ ensure reports are completed and submitted as required by ABC, NCCER, and the DOL; <br />■ provide support to instructors as needed; and <br />■ audit the training site to verify that all program guidelines, regulations, and instructional <br />practices are being met. <br />In -House Program, Costs <br />Costs forthe ABC Training Program to be held by Michiana Contracting are listed below. <br />Core/Level i — Cost per Student <br />New Student Application Fee $ 3.00.00 <br />■ Annual Tuition $ 560.00 <br />■ Textbooks $ 200.00 <br />(Instructor Textbooks - $1oo/copy as needed) <br />$ 860.00 <br />Level z —Cost per Student <br />■ Annual Tuition $ 560.00 <br />■ NCCERTextbooks $ 150.00 <br />(Instructor Textbooks - $1oo/copy as needed) <br />$ 710.00 <br />Program costs for each student are quoted as of January z, lois and may be subject to change. <br />Important Notice <br />Where program guidelines, regulations, and/ortraining conditions are not sufficiently met by <br />students, instructors, and/or the training facility, ABC of Indiana/Kentucky will drop all studIents with <br />the Department of Labor and immediately terminate the training program. <br />As additional students are added to a class, it is the responsibility of the training memberto ensure <br />ABC is notified and supplied with the appropriate information as listed above. <br />pg. 2 <br />
