INTER-OFFICE MEMORANDUM <br /> DEPARTMENT OF PUBLIC WORKS <br /> DIVISION OF ENGINEERING <br />TO: Anne Fuchs, Clerk <br />Board of Public Works <br />FROM: Zach Hurst, Project Engineer <br />SUBJECT: Change Order and Closeout Paperwork <br />120-051R Charles Martin Parking Lot <br />DATE: July 15, 2021 <br />The enclosed documentation contains necessary paperwork for closeout of the subject <br />project, including a Project Completion Affidavit and a Waiver of Lien. The amount of the <br />change order exceeds the 20% limit as set by Indiana State Code. The change order (final) is <br />explained below. <br />During the milling of the parking lot, it was discovered that several ‘soft’ areas of asphalt existed <br />under the parking lot, essentially having been worn down to fine graded soil beneath. Extra <br />asphalt was required to bridge over or fill these softer areas. While a quantity of approximately <br />20 tons was included in the original contract to account for areas like this, the total over-run was <br />40 tons, or roughly one extra truckload of asphalt. <br />Also during the milling operation, it was discovered the eastern drywell, outside the rear <br />entrance to the building, had failed and required full replacement. This required a new precast <br />concrete structure, and a separate pipe crew for the contractor to install. <br />Neither of the adjustments required to the contract could have been reasonably foreseen.
