Background Data: <br /> <br />a. <br /> <br />Effective Date of Owner-Engineer Agreement: <br /> <br />2/25/2020 <br /> <br />b. <br /> <br />Owner: <br /> <br />Board of Public Works, City of South Bend, Indiana <br /> <br />c. <br /> <br />Engineer: <br /> <br />VS Engineering, Inc. <br /> <br />d. <br /> <br />Project: <br /> <br />Dubail Ave. Streetscape <br /> <br />2. Description of Modifications: <br />a. The Scope of Services currently authorized to be performed by Engineer in accordance <br />with the Agreement and previous amendments, if any, is modified as follows: <br />1) Engineer shall perform additional schematic/conceptual design services to include <br />additional conceptual design options for consideration. Additional concepts include <br />a total of five options instead of the originally scoped three options. <br />2) Engineer shall, at the Owner’s request, re-design the 60% plan documents and 90% <br />plan documents per scope changes set forth from the Owner. Scope changes include <br />deviating from the full reconstruction of Dubail Avenue to designing only the <br />roadway intersections of Dubail Ave. with St. Joseph Street, Carroll Street, <br />Columbia Street, Fellows Street, Rush Street, Marietta Street, High Street, and Dale <br />Avenue. Design elements at these intersections include re-working the following <br />design elements: <br />a) Intersection geometrics <br />b) Construction detail sheets <br />c) Sidewalk design <br />d) ADA curb ramps design and detailing <br />e) Pavement markings <br />f) Signage <br />g) Storm water drainage design and computations <br />h) Erosion control plans <br />i) Maintenance of traffic plans <br />j) Quantity take off <br />k) Construction cost estimate <br />Each side of each intersection shall tie back into the existing Dubail Avenue <br />roadway and sidewalks. <br />
