• <br /> paid to each officer or sworn member regardless of whether <br /> the holiday is worked. <br /> (i) Clothing allowance. <br /> Officers and all other sworn members shall receive an <br /> annual clothing allowance of Seven Hundred Ninety-Nine <br /> Dollars ($799.00) , one-half (1/2) of which shall be paid on <br /> or before June 15th and one-half (1/2) of which shall be <br /> paid on or before December 15th. <br /> SECTION IV. OTHER MONETARY FRINGE BENEFITS. <br /> (a) Dependent Health Insurance. <br /> For 1989 and 1990, the bi-weekly contribution by <br /> the officers and sworn members for dependent health <br /> insurance costs for the City's self-funded health insurance <br /> plan shall not exceed Fifty Dollars ($50.00) per month. <br /> (b) Health Care Insurance Cost Reduction Program. <br /> For 1989 and 1990, the Board of Health Care <br /> Insurance Oversight, shall continue to operate in and <br /> advisory capacity to and under the direction of the City <br /> Controller. This Board shall be composed of representatives <br /> of the South Bend Firefighters Association, Local No. 362 <br /> and employees of the Department of Administration Finance of <br /> the City of South Bend. It shall be the responsibility of <br /> the Board to review, under the direction of the City <br /> Controller, on a regular basis, the activities of the City's <br /> Health Care Insurance Program. <br /> (c) 1989 Health Insurance Program. <br /> In addition, in an effort to control health care <br /> costs, the City's current health insurance package shall be <br /> modified to provide a program containing comprehensive major <br /> medical health insurance which would provide for first <br /> dollar after a One Hundred Dollar ($100.00) deductible per <br /> year is met by the sworm member or officer, deductibles of <br /> ten percent 10% of the cost of such major medical expense, <br /> with maximum claims annual cost to the sworn member or <br /> officer for such coverage being Five Hundred Dollars <br /> ($500.00) . <br /> The City shall pay Twenty Four Dollars ($24.00) <br /> per month to any sworn member or officer who elects to leave <br /> the City's insurance program to be covered by another <br /> program for which the City makes no contribution. This <br /> election shall not be mandatory and the sworn member or <br /> officer may return to the City's program without <br /> contribution of Twenty Four Dollars ($24.00) per month if <br /> and when the sworn member or officer loses such coverage or <br /> during open enrollment, provided that the conditions of the <br /> City's insurance plan are met. <br /> (d) 1990 Health Insurance Program. <br /> The City shall pay Twenty Four Dollars ($24.00) per <br /> month to any sworn member or officer who elects to leave the <br /> City's insurance program to be covered by another program <br /> for which the City makes no contribution.
