Call 811 or (800) 382-5544 for utility locates a minimum of 48 hours prior to <br />excavation. <br />2. The plumber or contractor must obtain all permits required by the City Engineering <br />Department. (excavation, street cuts, and sidewalk permits) <br />3. Call Engineering Department (574) 235-9251 a minimum of 48 hours prior to <br />excavation and street closure. <br />4. Call City Water Works Department at (574) 235-9279 for tap fee requirements. The <br />tap fee is to be paid to the Water Works Engineering Department, 830 N Michigan St. <br />5. Schedule taps through Field Operations number at (574) 235-9464. <br />6. Excavate, expose and clean water main for tap. <br />7. Material supplied by South Bend Water Works: <br />2" and smaller - corporation stop, curb stop and stop box <br />4" and larger - tapping sleeve and valve, roadway/valve box <br />8. All excavations must meet O.S.H.A. excavation standards. <br />9. Contractor shall provide all sheeting and shielding. <br />10. Domestic branches off of fire lines are strictly prohibited. <br />11. Any abandoned service lines within the property frontage of the new tap shall be <br />disconnected at the water main by the contractor during the tap excavation.
