(LFA) is pleased to submit this proposal to provide Professional Engineering Services to the City of South Bend (City) for the Main Street to Lafayette Boulevard Connector and Street Improvements. The proposal includes the following documents: • Scope of Additional Services, Appendix A • Manhour Justification, Appendix B The proposal was prepared at your request on the basis of our meetings on May 21, 2012 and September 20, 2012. It includes work items and work effort not envisioned or included in our original proposal. We propose this Scope of Services be performed on an hourly basis for a budgetary fee of $97,600.00. Payments to LFA will be in accordance with Attachment "A" in our original contract. Please indicate your acceptance of this proposal by signing in the space provided below and returning one copy to our office.
