The following forms of questionnaires and financial statement are prescribed <br />by the State Board of Accounts in conformity with the statute set out on the <br />preceding page. <br />These forms, properly filled out and attested, must accompany each bid of <br />five thousand dollars or more on any public work. <br />The forms are designed to cover all contracts for all kinds of work and the <br />bidder is required to answer such questions as are pertinent to the work upon which <br />his is bidding. The purpose of the questionnaire and financial statement, as set forth <br />in the law, is to enable the awarding body to determine the qualifications of the <br />bidder to carry out successfully the contract if the same is awarded to him. <br />The bidder will find it to his advantage to answer fully all questions coming <br />within the range of the work upon which he is bidding. Particular attention should <br />be given the "Financial Statement" and the details relative to the assets and liabilities <br />set out. This form is made in extensive detail to that the bidder may explain his <br />assets and liabilities in proper sequence and in a uniform manner. <br />T. M. HINDMAN, <br />State Examiner <br />
