Background Data: <br /> <br />a.Effective Date of Owner-Engineer Agreement: May 28,2019 <br /> <br />b.Owner: City of South Bend <br /> <br />c.Engineer:Jones Petrie Rafinski <br /> <br />d.Project:Downtown South Bend Wayfinding Phase II <br />2. Description of Modifications: <br />\[NOTE TO USER: Include the following paragraphs that are appropriate and delete those not applicable to <br />this amendment. Refer to paragraph numbers used in the Agreement or a previous amendment for clarity <br />with respect to the modifications to be made. Use paragraph numbers in this document for ease of <br />reference herein and in future correspondence or amendments.\] <br /> <br />a. Engineer shall perform or furnish the following Additional Services: <br />1) Conduct site visits with City staff to identify locations of additional wayfinding <br />signage. <br />2) Prepare the plans and special provisions for the additionalwayfinding signs. <br />Construction documents are to include foundation designs and location plans. <br />These details will be based on the previously developed sign designs. <br />3) Prepare bid documents for the kiosk and wayfinding signincluding the foundation <br />design, fabrication and installation of the proposed signs, and minor electrical <br />services to select lit signs. <br />4) Phase II signs are to include the following: <br />i. 2 lighted gateway welcome to SB signs <br />ii.22 vehicular multi-message guides (19 on decorative poles, 3 on INDOT <br />beams) <br />iii.10 vehicular trailblazers (8 on decorative poles, 2 on INDOT beams) <br />iv.2 vehicular trailblazers on existing poles <br />v. 2 pedestrian kiosks <br /> <br />b.
