II. Pre-Qualification Checklist (for bidders thatare not pre-qualified) <br /> <br />(b) Attachments <br /> <br />(i) Nelco Media, Inc. will apply for certificate of good standing with the Indiana Secretary <br />of State if awarded contract. <br /> <br />(ii) No former business names. <br /> <br />(iii) No violations of federal, state or local laws. <br />(iv) Our labor sources include all necessary staff for the completion of the scope of <br />work. <br />(v) All individuals that will perform work on the public work project on our behalf will be <br />properly classified as an employee or as an independent contractor under all applicable <br />state and federal laws and local ordinances. <br />(vii) All employees will be tested in accordance with the requirements set forth in IC 4- <br />13-18-5 or IC 4-13-18-6. <br /> <br />(viii) Nelco Media will be utilizing a surety company from the “Department of Treasury’s <br />Listing of Approved Sureties” if awarded contract. <br /> <br />(ix) No federal, state or local tax liens or tax delinquencies. <br /> <br />(x) City of McAllen, Texas studio build-out. City of Lawrence, Kansas City Council <br />Chambers a/v upgrade. City of San Antonio, Texas City Council a/v upgrade and studio <br />build-out. Texas School for the Deaf, Austin, Texas complete studio build-out and live <br />streaming/broadcasting upgrades. <br /> <br />
