4)Privet Lane Topographic Survey (COMPLETE): JPR was requested to prepare a <br />topographic survey for the failing structure locatedon Privet lane. <br />5)Laurel Woods Structure Survey (COMPLETE): JPR was requestedtocollect <br />existing structure data for variousstructures in the Laurel Woods area for <br />determining issues with existing stormdrainage. <br />6)Nimtz Parkway Boundary Survey/EasementPreparation (CURRENT/ACTIVE): <br />JPR was requested to prepare a boundary survey and easement descriptionfor a <br />City owned parcelfor the Nimtz Parkway Retention Basin. This is an active <br />project. <br />7)Drainage Request areas(CURRENT/ACTIVE):topographicsurveys requested for <br />the followingareas to correct drainage issues:MarionStreet (Williams St.to <br />Lafayette Blvd.),Fox Street (High St. toDaleAve,),Beale Street (Inglewood Pl. to <br />Sherman Ave.), Huey Street (Pershing St.to Bergan St.), Elmer Street (Pershing St. <br />to Bergan St.), and Kaley Street(Right-of-way to drainage ditch) <br />b.The Scope of Services currently authorized to be performed by Engineerin accordance <br />with the Agreement and previous amendments, if any, is modified as follows: <br />c.The responsibilities of Owner are modified as follows: <br />d.For the Additional Services or the modifications to services set forth above, Owner <br />shall pay Engineer the following additional or modified compensation: <br />Construction Documents$21,500.00 <br />Total Additional Services Request$21,500.00 <br />e.The schedule for rendering services is modified as follows: <br />f.Other portions of the Agreement (including previous amendments, if any) are modified <br />as follows: <br />3.Agreement Summary (Reference only) <br />a.
