Wayne Street <br />South Bend, IN <br />Re: Masonry Restoration Inspection <br />Dear Hickeys: <br />t � . <br />ROOFING AND RESTORATION, INC. <br />1703 South Ironwood - South Bend, IN 46613 <br />Phone 574/233-7485 <br />Fax 574/233-7531 <br />slatile@comcast.net <br />A masonry inspection was made on October 21, 2014 at the above address, and the following <br />was noted: <br />1. The condition of the steel lintels was poor and failing over approximately 75% of the <br />window openings, including the basement windows. <br />2. A severe crack of the brick on the west elevation, where the TV antenna tower is <br />attached, was noted. <br />3. The brick soldier course over the entire length of the foundation had a continuous crack <br />on the east elevation with a few loose brick. <br />4. The northeast corner of the house had a severe crack, and actually shifted, as if the <br />house were struck by some piece of equipment. <br />5. <br />6. The garage had a few cracks in the mortar as well. <br />Slatile suggests the following corrective measures be taken. <br />1. The removal of the TV antenna tower, and repair cracked wall. <br />2. Removal and replacement of the 20 failed steel lintels, and repair associated brick <br />cracks. <br />3. Removal of the small 30" long gutter on the west elevation that has no downspout. <br />4. Removal of the mortar of the structural crack on the northeast corner of the house and <br />replace with new (i.e., tuckpoint). <br />Slatile proposes to perform the above described masonry repairs for the sum of $26,580.00. <br />If the basement windows are damaged beyond repair, we suggest to remove them and replace <br />with glass block, add the sum of $3,675.00 <br />This work does not include any interior repairs such as plaster patching or painting. <br />V <br />
