As I have been unable to reach you by phone, I am sending this letter. The <br />information you submitted is insufficient to process your request. Please complete the enclosed <br />application form and submit it to the address above. Please complete one application for all three <br />proposed projects. Indicate final construction methods; materials, amount of excavation and/or <br />fill (cubic yards and area), and any re -vegetation plans in the application. <br />IDEM is required to public notice all applications for WQC. Please include appropriate <br />contact names and addresses per the application. Your application for WQC will be held in <br />abeyance until a complete application is received.
