Pursuant to a survey by the Indiana Department of Health and Human Services, Health Care <br />Financing Administration, of 26 January 1999, the house owned and operated by LOGAN as a <br />group home located at 1215 North Riverside Drive, South Bend, IN, the following citation of <br />deficiency regarding the Life Safety Code was issued. This survey was conducted by one Robert <br />Booher, Life Safety Code Specialist. <br />The citation was based upon the following K 032, NFPA 101 Standard: Life Safety Code <br />Standard: <br />At least two acceptable exits that do not involve windows and remote <br />from each other are provided, at least one path of travel shall be provided that <br />does not traverse any space exposed to unprotected vertical openings, or <br />common living spaces. 21-2:2.3.3, - <br />This STANDARD is not met as evidenced by: <br />1. Based on observation and interview, the facility failed to insure 1 of 5 <br />sleeping rooms was provided with two exits which did not involve windows, <br />and at least one exit with a path of travel that did not traverse any space <br />exposed to common living areas such as living rooms, dining rooms and <br />kitchens. <br />Findings include: <br />A. Based upon observation at 3:18 p.m. on 1.26.99, the only exit from the <br />first floor sleeping room that did not involve the use of the window was to <br />traverse the short corridor that exited into the dining room, a common living <br />space. From the dining room the client could go into the living room, and <br />exit using the front door, or into the kitchen and exit through the back door. <br />Per interview with the Supervised Living Manager at 3:18 p.m. on this same <br />date, the client sleeping in the first floor sleeping room has a physician's <br />order to be on the first floor due to seizures. As a result, it could not be <br />assured this client in the first floor sleeping room would be able to move to <br />safety during a fire. <br />In order to accommodate the above citation and have it removed from the record of the house <br />located at 1215 North Riverside Drive, South Bend, IN, LOGAN proposes to the South Bend <br />Historical Development Commission the following request: <br />To cut a doorway directly from the downstairs bedroom to the exterior of <br />the house on the north side, first floor (please see attached photographs <br />with markings as to location of the doorway). <br />The doorway would be cut through the exterior wall of the house and a stoop <br />approximately 48 inches by 48 inches complete with banister around the stoop <br />with steps leading to the ground surface complete with hand railings. <br />
