SUBSTANCE ABUSE POLICY DOCUMENT No.: EHS-000146 REV #: 1 <br />Date of Issue: 812312016 Next Rev.: <br />EH&S Procedures Issued By: Corporate Safety Director 6/15/2021 <br />Date of Approval: 6/15/2017 <br />Standard Owner: EH&S Department Annroved Bv: SafetV <br />Appendix — B: <br />Explanation of Reasonable Suspicion and <br />Post Accident / Incident Testing <br />Reasonable Suspicion Testing: <br />If an employee exhibits inappropriate behavior, demonstrates performance problems or there is <br />other evidence to suspect an employee is under the influence of controlled substances, (See <br />Attachment A — Reasonable Suspicion Report, to assist and document) the employee may be <br />subject to a substance abuse screen/test. <br />If the employee is requested to be tested either on the job site or refuses to submit to such a <br />substance abuse test, they will be disciplined the same as if they had tested positive. <br />Disciplinary action based on a positive result or refusal to submit to a test will be in compliance <br />with the Federal/State Regulations and the Company policy. <br />Employees shall submit to drug/alcohol screening where there is cause to believe this policy <br />has been violated. Examples of sufficient cause include, but are not limited to: <br />a. Known possession of controlled substances. <br />b. Involvement in a drug related incident. <br />c. Employees exhibiting drug or alcohol symptoms. <br />• Lack of balance, unsteady or staggered walk or muscle twitching. <br />• Red, watery or glassy eyes, dilated or constricted pupils or having difficulty focusing eyes <br />• Disorientation, uncoordinated movement of body and hands. <br />• Face/cheeks flushed, face sallow. <br />• Slurred speech, memory loss or drowsiness. <br />d. Has violated the substance abuse policy or work rules. <br />e. Has sustained a personal injury or has caused another person to sustain a personal injury. <br />f. Has caused a work -related accident or was operating or helping to operate machinery, <br />equipment, or vehicles involved in a work -related accident or incident. <br />To assist supervision further in the determination of a reasonable suspicion related case; <br />(Attachment B) contains guidelines. <br />Post Accident and Incident Testing: <br />Testing will be required following an on-the-job injury requiring medical treatment or following a <br />potentially serious incident, including near misses, in which safety precautions were violated, <br />unsafe instructions, or orders were given, potentially serious accident occurred where <br />vehicles/equipment/property was damaged, unusually careless acts were performed, or where <br />the cause was due to failure to wear prescribed personal protective equipment. <br />Page 14 of 22 <br />
