City of South Bend Project No. 120-021Change Order No. 105/12/20 <br />OWNER:City of South Bend <br />ENGINEER:City of South Bend Engineering <br />Alicia Czarnecki, Project Engineer <br />CONTRACTOR: Selge Construction Co., Inc. <br />PROJECT:Hydrant Relocation Quote <br />Project No. 120-021 <br />SUBJECT:Change OrderGreater than 20% <br />ISSUE Date:May 05, 2020 <br />The following is a summary and descriptionof Change OrderNo.1: <br />The original intent of this project was to close existingwater valvesand relocate hydrant <br />assemblies inanticipation of twoupcomingINDOT projects, Corby/Ironwood/Rockne <br />Intersection Improvements and Olive Street Reconstruction.Most ofthe work was tobe <br />completedin the tree lawn area. <br />On Olive Street, closing the valves became a much more difficult task due tothevalves <br />being completely paved over, aswell asthe proximityto a proposed inlet for the upcoming <br />INDOT project which would put the relocated lines directly in conflict with the inlet. To <br />addressthis,a larger pavement section was removed,andanInserta Valvewas installed <br />closer to the mainline.This allowed the contractor to install two elbows out of the way of <br />the proposed structure. <br />This work was performed ona time and material basis due to the unforeseensite <br />conditions atthis projectlocation.As a result of the unforeseenwork related to closing the <br />existing valves, the change order amount is$16,856.79, which is 36.75%.
