Quality workmanship, efficient operation, safety, and timely completion of projects requiresthat all bidders meet certain minimum requirements to be responsive and responsible bidders. THIS FORM MUST BE SUBMITTED WITH YOUR BID. INSTRUCTIONS: If you are a preIf you are a pre--qualified bidder, complete Section I only. qualified bidder, complete Section I only. If you are not a pre-qualified bidder, complete Section II only. Section II acts as an application for pre-qualification. Submission of Section IIwill allow the bidder to be considered for pre-qualification for bids with the City of South Bend Department of Public Works. Pre- qualified bidders will then be exempt from a portion of thesubmission requirements outlined in Section 6- 63 of The ResponsibleBiddingOrdinance No. 10594-18 for a period of twelve (12) months. Thereafter, contractors who are pre-- rov-Qualified st for the upcoming calendar year, or within twelve (12) months of obtaining pre- qualified standing. If the status of any item changes within the twelve (12) months,it is the responsibility of the contractor to notify the City.
