Background Data: <br />a. Effective Date of Owner -Engineer Agreement: <br />b. Owner: Board of Public Works. Citv of South Bend, Indiana <br />C. Engineer: American Structurepoint Inc. <br />d. Project: Morris Center of Performing Arts Emergency Plaster Work <br />2. Description of Modifications: <br />[NOTE TO USER: Include the following paragraphs that are appropriate and delete those not applicable to <br />this amendment. Refer to paragraph numbers used in the Agreement or a previous amendment for clarity <br />with respect to the modifications to be made. Use paragraph numbers in this document for ease of <br />reference herein and in future correspondence or amendments. <br />a. Engineer shall perform or furnish the following Additional Services: <br />American Structurepoint will provide the construction administration services <br />required to assist the City of South Bend required to complete the plaster repairs <br />at the Morris Performing Arts Center. <br />b. The Scope of Services currently authorized to be performed by Engineer in <br />accordance with the Agreement and previous amendments, if any, is modified as <br />follows: <br />• Provide one site visit to inspect plaster installation mock-up at plaster <br />contractor's facility with follow-up report of findings. <br />• Provide no more than two inspections of the dome plaster to determine the extent <br />of repairs required. For the purpose of this proposal it is assumed one inspection <br />will occur during scaffold installation to keep on schedule and another <br />inspection once scaffold construction is complete. <br />• Communication and coordination as required with contractors during repairs. <br />• Provide one final inspection of repairs upon completion. <br />e.
