(5) Violates the conditions of this ordinance or any <br /> final judicial order entered with respect thereto . <br /> (1) Notification of violation; administrative adjustment . <br /> Whenever the Board of Public Works finds that a permittee has <br /> pursuant to this code, the Board shall s-crvc or cause—to—be served <br /> upon such permittee a written notice by certified mail, return <br /> receipt requested, stating the nature of the alleged violation. <br /> permittce shall respond in writing to the Board of Public Works <br /> advising of its position with respect to the allegations . <br /> violations . <br /> (m) Show cause hearing. Where the violation of this code is <br /> adjustment, the Board of Public Works may order any discharger which <br /> Board or its duly authorized eprest_ntive e prepos-ed—permit <br /> revocation should not be taken. The notification shall also <br /> summarize any enforcement actions taken against the dischargers <br /> during the same twelve (12) months . <br /> (k) Revocation of Permit . The Board of Works may revoke the <br /> permit of any user that violates the following conditions of this <br /> ordinance, wastewater discharge permit, or orders issued hereunder. <br /> (1) Violation of wastewater discharge permit <br /> conditions . <br /> (2) Failure to accurately report the wastewater <br /> constituents and characteristics of its discharge. <br /> (3) Failure to report significant changes in <br /> operations or wastewater volume, constituents and <br /> characteristics prior to discharge. <br /> (4) Refusal of reasonable access to the user' s <br /> premises for the purpose of inspection, monitoring or <br />
